Lifetime rewards never received

I've hit the milestone for 4 out of the 5 lifetime rewards and filled out the forms each time, but I'm still waiting on them. It's been months now, and I'm starting to wonder if there's a glitch in the system or if I've missed something. Has anyone else experienced similar delays? Any advice or insights would be greatly appreciated.

    All glitches are designed to benefit the company. "Oops" is something it has little trouble saying, since the table is slanted toward them at all times.
